So what is an holistic removal of amalgam fillings? Is the safe removal of Mercury Amalgams using a high-powered oral vacuum to reduce the mercury exposure and other toxic vapors. This safety precautions are known as the S.M.A.R.T. Protocol.

Mercury Amalgams are a combination of metals including: silver, copper and tin. The proportion of mercury consists on over 50% because it helps all these metals combine together.

S.M.A.R.T. PROTOCOL

The IAOMT protocol recommendations for amalgam removal are known as the Safe Mercury Amalgam Removal Technique (SMART).

1. Reduce mercury vapors by cooling with water

All removal must be done under copious amounts of cool water to reduce heatand a conventional high speed evacuation device to capture mercury discharges are required to reduce ambient mercury levels.

2. Reduced drilling of the amalgam

The amalgam needs to be sectioned into chunks and removed in as large of pieces as possible using a small diameter carbide drill. By cross hatching the material and removing bulk pieces.

3.High volume suction with custom isolation tip (Clean Up brand)

High speed evacuation produces better capture when fitted with a Clean Up device,45, 87 which is not mandatory but is preferred..

4. Secondary air evacuation

All patients having amalgam removed or placed should be provided with an alternative air source and instructed to not breathe through their mouth during treatment. A nasal hood such as is used with the nitrous oxide analgesia equipment is excellent. Air is best and oxygen is acceptable although not required. If just air is used it should be clean and free of mercury vapor preferably from outside the dental office.

5. Use a rubber dam

Dental dam that is made with non-latex nitrile material should be placed and properly sealed in the patient’s mouth. Patient saliva suction behind the rubber dam.

6. Use of Non-latex gloves

Non-latex nitrile gloves must be utilized by the dentist and all dental personnel in the room.

7. Immediately clean up

After the fillings have been removed and replaced, the dentist or dental assistant should immediately remove and dispose of the patient’s protective covering and thoroughly clean his face and neck. All mercury safe dentists should routinely do this but remind the assistant if she forgets, after all you don't want to take any mercury home with you.

8. Use of Charcoal

Once the removal process is complete, the patient’s mouth should be thoroughly flushed with water and then rinsed out with a slurry of charcoal, chlorella or similar adsorbent.

9. Filtering air in the operatory

Each room where mercury fillings are removed must have adequate filtration in placewhich requires a high-volume air filtration system (such as an at source oral aerosol vacuum) capable of removing mercury vapor and amalgam particles generated during the removal of one or more mercury fillings.

10. Provide Clean Air for the Patient

External air or oxygen delivered via a nasal mask for the patient also should be utilized to assure the patient does not inhale any mercury vapor or amalgam particulate during the procedure. A nasal cannula is an acceptable alternative for this purpose as long as the patient’s nose is completely covered with an impermeable barrier

11. Protect Patient Skin and Clothing

A full body, impermeable barrier, as well as a full head/face/neck barrier under/around the dam, should be utilized

THE IAOMT PROTOCOL Reccomendations

Face shields and hair/head coverings are to be utilized by the dentist and all dental personnel in the room

Either a properly-sealed, respiratory grade mask rated to capture mercury or positive pressure, properly-sealed mask providing air or oxygen should be worn by the dentist and all dental personnel in the room

In order to protect the patient’s skin and clothing, a full body, impermeable barrier, as well as a full head/face/neck barrier under/around the dam, should be utilized

A dental dam that is made with non-latex nitrile material should be placed and properly sealed in the patient’s mouth

A saliva ejector should be placed under the dental dam to reduce mercury exposure to the patient

During amalgam filling removal, the dentist should utilize an at source oral aerosol vacuum in close proximity to the operating field to mitigate mercury exposure

The amalgam should be sectioned into chunks and removed in as large of pieces as possible, using a small diameter carbide drill

Alternatives to Silver Fillings

Because of Zirconia translucent properties this material is the most similar to natural teeth.

Zirconia is a good choice aesthetically speaking the durability of the color of this material is better than a porcelain or ceramic restoration.

Zirconium wear is minimal compared to other materials.
